Our team of healthcare professionals is board certified nurse practitioners specializing in geriatric and adult medicine. We provide our patients with comprehensive evaluations of all body systems and create a plan of care based on individual needs. Our practice follows a patient centered model of care with a personalized review of the patient’s medical, social and spiritual needs. We also provide short-term services which are available upon request by the patient’s regular physician. Our goal is to provide services to those who can no longer travel outside the home on both a permanent and temporary basis as required.
